29 CFR 1903.19(c)(3): The employer did not submit certification that abatement is complete for each cited violation, in addition to the information required by paragraph (h) of this section, the date and method of abatement and a statement that affected employees and their representatives have been informed of the abatement.  On or about November 12, 2015, the employer failed to submit an abatement certification for the following citation issued on October 23, 2015;  1. Citation 1, Item 1: 29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  Location - 206 Ridge Street, Winchester, MA:  On or about 6/12/15, the employer did not ensure that the scaffold was constructed and designed by a qualified person to recognize the hazards associated with the home-made wooden scaffold.
