29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    a) 624 5th Street, Philadelphia, PA - The employer did not protect employees from falling through holes of the staircase more than six (6) feet above lower levels by personal fall arrest systems, covers or guardrails during demolition and construction of residences.  Employees were exposed to falls of approximately eleven (11) feet. Observed on or about June 19, 2015.
