29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:    a/  Inside of the Shipping Department, a yellow, metal fabricated personnel basket was purchased and installed onto the forks belonging to a Komatsu Fork Truck, m/n FB25811-6, s/n 90622A.  Written permission or the equivalent was not obtained from the manufacturer of this fork truck prior to this action.
