29 CFR 1910.37(b)(2): Each exit must be clearly visible and marked by a sign reading "Exit."  (a) On or about June 22, 2015, the employer failed to mark the exit from the office with a sign reading "Exit." This room has two doors. One leads outside and the other leads within the building. In the event of a fire, employees would be exposed to fire hazard.

29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body must be provided within the work area for immediate emergency use.   (a) On or about June 22, 2015, the employer failed to provide suitable facilities in the immediate area available for quick drenching or flushing of the eyes and body. Employees were exposed to corrosive sulfuric acid in batteries, in that there were no suitable facilities in the immediate area available for quick drenching or flushing of the eyes and body.

29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings must be closed to afford protection substantially equivalent to the wall of the equipment.  (a) On or about June 22, 2015, the employer failed to close unused openings in the electrical panel next to the door on the east wall of the building. Employees were exposed to risk of fire in the event of an arc flash because the panel had four (4) uncovered openings. On or about June 22, 2015, the employer failed to close an unused knockout in the top of the electrical panel next to the door on the east wall of the building. Employees were exposed to risk of fire in the event of an arc flash due to the uncovered opening.  (b) On or about June 22, 2015, the employer failed to close an unused knockout in the top of the outlet box behind the vending machine next to the office door in the interior of the building. Employees were exposed to risk of fire in the event of an arc flash due to the uncovered opening.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings must be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  (a) On or about June 22, 2015, the employer failed to provide an electrical outlet box in the office with a faceplate. Employees were exposed to risk of fire in the event of an arc flash due to the uncovered opening.

29 CFR 1910.305(g)(1)(iv)(A): As a substitute for the fixed wiring of a structure;  (a) On or about June 22, 2015, the employer failed to use fixed wiring and instead used damaged flexible cords and cables as a substitute for the fixed wiring of a structure in that three extension cords were connected in succession in order to supply power to the tool bench area. Employees were exposed to risk of fire and electric shock.
