29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Facility, 1600 Washington Street, Specialty Tire Department, Indiana, PA: On or about 06/18/15 a method of machine guarding was not maintained on the material feed section on the 2nd stage radial machine #2. The electronic eye used as a method to control the movement(s) of the roll feeders had been covered and did not provide guarding as it was intended.
