29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered.   a)  Maintenance Yard, On or about 6/19/15: An employee was operating a John Deere Skid Steer, Model 250 without using the  seat belt. The skid steer  experienced an overload situation  causing  the unseatbelted operator to be thrown forward which exposed his hand to the pinch point created between the tailgate of the dump truck and the cage of the skid steer. This action resulted in the operators left ring finger to be severed above the knuckle joint.
