29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment:   On or about June 23, 2015, employees were seen working in the lab room with patient specimens, such as but not limited to, blood specimens without eye or face protection nor lab coats to protect clothing from accidental splashes of bodily fluids.

29 CFR 1910.1030(c)(1)(i): The employer had employees with occupational exposure and did not establish a written Exposure Control Plan designed to eliminate or minimize employee exposure:  On or about June 23, 2015, at this children's clinic, employees were involved with patient care, such as but not limited to, taking blood and urine specimens and giving vaccinations exposing employees to potentially infectious materials. The employer did not have a written Exposure Control Plan in place.
