29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support): Worksite: When using non-self supporting ladders to access the second story of a house, the employer set the ladders at an angle where the horizontal distance from the top support to the foot of the ladder was greater than one-quarter of the working length of ladder. This caused employees using this ladder to be exposed to fall hazards to the ground below.

29 CFR 1926.1060(a): The employer shall provide a training program for each employee using ladders and stairways, as necessary. The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards. Worksite: Employees using ladders on site were not trained to recognize hazards related to improper ladder erection and maintenance. At least one ladder was set an angle such that the horizontal distance from the top support was greater than one-quarter of the working length of the ladder. This exposed employees to a fall to the ground below when accessing the ladder to perform painting activities.
