29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts: a) Machine Shop, on or about 6/23/15: Enco Lathe Model III-3540 the rotating chuck was not guarded. b) Machine Shop, on or about 6/23/15: Rockwell Drill Press was not provided with a chuck and tool guard. c) Machine Shop, on or about 6/23/15: Bridgeport Mill was not provided with a chuck and tool guard. d) Machine Shop, on or about 6/23/15: Wells Metal Cutting Band Saw was not protected with a guard on the unused portion of the blade on both the right and left side. e) Machine Shop, on or about 6/23/15: Caterpillar pipe threading machine was activated by a manually operated on/off switch that would not automatically shut the machine off should the operator be inadvertently caught up in the rotating parts. f) Maintenance Shop, on or about 6/23/15: Jet Drill Press was not provided with a chuck and tool guard. g) Triple Stack Pre-Heater Area, on or about 6/23/15: Three rotating breaks on the Triple Stack Pre-Heater were not guarded.
