29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition: On or about June 23, 2015, employees were exposed to slip hazards when there was an almost constant flow of water on the floor from a leak in the ceiling that flowed down the wall to a floor drain. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: On or about June 23, 2015, employees were exposed to fire hazards when a. one fire extinguisher was being used as a door stop b. one fire extinguisher was mounted under a counter in the employee customer service area without signage.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: On or about June 23, 2015, the employer had not trained the employees where to find the SDS, what type of information was available on an SDS such as, but not limited to, recommended personal protective equipment and whether or not the chemical can cause illnesses. The employees used chemicals such as, but not limited to, Kay Solidsense Sanitizer and Kay All Purpose Super Concentrate. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
