29 CFR 1910.132(a): Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    a)  On or about June 24, 2015, during cutting activities on the Hobart Meat Saw, located in the butcher area, the employer did not provide, allow, and maintained personal protective equipment for the hands when employees were exposed to cut and amputation hazards.    b)  On or about June 24, 2015, during cutting activities on the Hobart Deli Meat Slicer, Serial Number 56-899-019, Model #1712, located in the deli area, the employer did not provide, allow, and maintained personal protective equipment for the hands when employees were exposed to cut and amputation hazards.    c)  On or about June 24, 2015, during frying activities on the Henny Penny Pressure Fryer Model 500, Serial #HPE located behind the deli area, the employer did not provide, allow, and maintained personal protective equipment for the hands when employees were exposed to burn hazards.    d)  On or about June 24, 2015, during cutting activities with fruits and vegetables, the employer did not provide, allow, and maintained personal protective equipment for the hands when employees were exposed to cut and amputation hazards.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a)  On or about June 24, 2015, during cleaning work and sanitation activities, the employer had not developed a written hazard communication program which met the specific outline criteria for safety data sheets and employee training requirements.
