29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:  At the project, eye protection was not used while using hammers to tear drywall from the ceiling.  Employees were exposed to the hazard of being struck in the eyes by the drywall debris.

29 CFR 1926.403(i)(2)(i): Except as required or permitted elsewhere in this subpart, live parts of electric equipment operating at 50 volts or more was not guarded against accidental contact by cabinets or other forms of enclosures, or by any of the following means:  Inside the store, live parts of electric equipment operating at 110 VAC was not guarded against accidental contact by cabinets or other forms of enclosures, as a receptacle was removed from the outlet box and was hanging by the electrical wires from the box to the receptacle.  Electrical tape was wrapped around the terminals on the outlet.  Employees working around the outlet with ladders and tools were exposed to the hazards of contacting electricity.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief was provided which will prevent pull from being directly transmitted to joints or terminal screws:  Inside the storage room, flexible cords were not connected to devices and fittings so that strain relief was provided, as the outside insulation was pulled back on the receptacle on a flexible cord that was used to provide power to a Milwaukee circular saw.  Employees that used the flexible cord and saw to cut lumber were exposed to the hazards of contacting electricity.
