29 CFR 1910.23(a)(5): Every pit and trapdoor floor opening, infrequently used, was not guarded by a floor opening cover of standard strength and construction. While the cover is not in place, the pit or trap opening shall be constantly attended by someone or shall be protected on all exposed sides by removable standard railings: a) Exterior, North Garage Air In-Take Shaft - A protective security officer who walked across a grating system located over the air intake shaft was exposed to a trip and fall hazard, on or about, 06/13/2015. Not all the security clips were fastened properly on the system and seven of the grating sections were elevated, . In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: a) Post 18A at North Garage Air In-Take Shaft - A protective security officer was observed exposed to a 9 ft. 10 in. fall hazard while performing patrolling duties standing above the north garage entrance, on or about 06/14/2015. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
