29 CFR 1926.403(b)(1): Examination. The employer shall ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined on the basis of the following considerations: 29 CFR 1926.403(b)(1)(iii): Electrical insulation. a. On the energized electrical cord used to supply power to the Target Masonry Saw, the insulation was cut and wires were sticking out of the insulation thereby exposing employees to a electrical hazard.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. a. On the front of the building where masonry crews were using a masonry saw to cut block, the electrical cord was pulled out of the back of the motor and did not have strain relief thereby exposing employees to an electrical hazard.
