You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. a) At the establishment, on the OSHA 300 log for 2015, a recordable case occurring on 04/27/15 resulting in 1 day away from work did not describe the injury to the employee. Column "F" must include a description of the injury, the parts of the body affected, and the object/substance that directly injured or made the person ill. b) At the establishment, on the OSHA 300 log for 2015, a recordable case occurring on 05/06/15 resulting in 7 days away from work did not describe the injury to the employee.
