29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end. On or about June 25, 2015, during the instillation of siding materials at a job site located at 672 Layaffette, Medina, Ohio 44256, the employer failed to failed to protect the employees from an approximately 11 foot fall by having the platform end extend over 18 inches past its support without guardrail protection.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. On or about June 25, 2015, during the instillation of siding materials at a job site located at 672 Layaffette, Medina, Ohio 44256, the employer failed to protect his employees from a fall of approximately 14 feet by using a guard rail or personal fall arrest system while working from a pump jack scaffold.

29 CFR 1926.451(d)(3)(i): Before the scaffold is used, direct connections shall be evaluated by a competent person who shall confirm, based on the evaluation, that the supporting surfaces are capable of supporting the loads to be imposed. In addition, masons' multi-point adjustable suspension scaffold connections shall be designed by an engineer experienced in such scaffold design. On or about June 25, 2015, during the instillation of siding materials at a job site located at 672 Layaffette, Medina, Ohio 44256, the employer failed to assign a competent person to ensure the supporting surfaces were capable of supporting the loads to be imposed when they used the porch roof to support the scaffold platform.
