29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about June 18, 2015, at the Ameren Hillsboro Gas Storage Field, protective equipment, such as hydro blasting footwear and suit, were not used when an employee performed a 6000 psi water blasting operation inside a confined space vessel.
