29 CFR 1926.102(a)(1): Employees must be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.  (a) Worksite located at 202 Grand Blvd, Kansas City Missouri 64106. Employees were applying a sealant onto a concrete wall prior to painting without safety glasses to protect the employee's eyes.

29 CFR 1926.403(b)(1): The employer must ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment must be determined on the basis of the following considerations:  (a) Worksite located at 202 Grand Blvd, Kansas City, Missouri 64106. The employer failed to keep a safe work environment free from electrical hazards when a drill and an electrical cord were placed in a wet environment. The employees were exposed to shock and electrocution hazards when a drill plugged in was placed in the vicinity of water.

29 CFR 1926.453(b)(2)(iv): Employees must always stand firmly on the floor of the basket, and must not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position.    (a) The worksite location at 202 Grand Blvd, Kansas City, Missouri 64106. The employer failed to provide a fall arrest system for the employee working in an aerial lift above ten feet off of the ground. The employer failed to provide training for the employee on maintaining two feet on the floor of the basket at all times. The employee was exposed to a fall hazard in excess of 30 feet.

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.  Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).    (a) The worksite location at 202 Grand Blvd, Kansas City, Missouri 64106. The employer failed to provide a fall arrest system for the employee working in an aerial lift above ten feet off of the ground. The employee was exposed to a fall hazard in excess of 30 feet.
