29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:       a) Warehouse - The employer did not obtain the manufacturer's written approval prior to modifying the Toyota sit down forklifts by attaching the Cascade clamps.  The data plates on the Toyota forklifts were not marked to identify the attachments and did not show the capacity weight of the truck and attachment combination.

The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor:    a) Warehouse- The employer did not ensure that all exit routes were kept free and unobstructed.  The exit route and access to the emergency exit door located to the corner of the warehouse was obstructed by material, including a broom, rake, garbage can, carpet rolls, wood boards, and equipment.
