29 CFR 1910.23(a)(2): Every ladderway floor opening or platform was not guarded by a standard railing with standard toeboard on all exposed sides (except at entrance to opening), with the passage through the railing either provided with a swinging gate or so offset that a person could not walk directly into the opening:    a) At the Gas-Well Gas Cryogenic Skid, employees, who climbed the south fixed ladder to access valves and check the sight-glass, were exposed to the hazard of falling more than 11-feet to the lower level.  The passage through the railing around the ladder platform was not provided with a swing gate nor was it off set so that an employee could not walk directly into the opening.    b) At the Gas-Well Gas Cryogenic Skid, employees, who climbed the north fixed ladder to access the valve to isolate the cold box side from the dehy side, were exposed to the hazard of falling more than 13-feet to the lower level. The passage through the railing around the ladder platform was not provided with a swing gate nor was it off set so that an employee could not walk directly into the opening.    c) At the Raw Product Surge Tank South, employees, who climbed the fixed ladder to access valves, were exposed to the hazard of falling more than 14-feet to the lower level. The passage through the railing around the ladder platform was not provided with a swing gate nor was it off set so that an employee could not walk directly into the opening.    d) At the Raw Product Surge Tank North, employees, who climbed the fixed ladder to access valves, were exposed to the hazard of falling more than 14-feet to the lower level. The passage through the railing around the ladder platform was not provided with a swing gate nor was it off set so that an employee could not walk directly into the opening.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level was not guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there was entrance to a ramp, stairway, or fixed ladder:    a) At the Gas-Well Gas Cryogenic Skid, employees, who accessed the north elevated work platform, were exposed to the hazard of falling more than 13-feet to the lower level. The southeast corner of the platform had an opening in the guardrail system that was approximately 37 5/8-inches wide.  The attachment point for the removable chain top-rail was broken, and the removable mid-rail chain was missing.    b) At the Raw Product Surge Tank South, employees, who accessed the elevated work platform, were exposed to the hazard of falling more than 14-feet to the lower level. The northwest corner of the platform had an opening in the guardrail system of more than 36-inches wide.      c) At the Raw Product Surge Tank North, employees, who accessed the elevated work platform, were exposed to the hazard of falling more than 14-feet to the lower level. The southwest corner of the platform had an opening in the guardrail system of more than 36-inches wide.      d) At the V-210 Storage Tank, employees, who accessed the elevated work platform, were exposed to the hazard of falling more than 14-feet to the lower level. The northwest corner of the platform had an opening in the guardrail system through which an employee could pass.
