29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as follows:   a) Jobsite, at or near 2895 West Liberty Avenue, Pittsburgh, PA: On or about June 23, 2015, during stonework related construction activities, the employer did not ensure that all scaffold working levels were fully planked in that along the top of one section of scaffold, there was no planked deck due to the presence of a telephone pole that was located inside of where the scaffold had been set up.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface were not used:    a) Jobsite, at or near 2895 West Liberty Avenue, Pittsburgh, PA: On or about June 23, 2015, during stonework related construction activities, the employer did not ensure that a safe means of access/egress to or from the scaffold was in use, exposing employees to fall hazards of up to approximately 13 feet when getting on or off of it.
