Employees were exposed to fall hazards as observed on or about June 25, 2015.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) At the jobsite: A 32-foot Werner portable extension ladder was used to access a residential roof which was about 22 feet high from the ground level and the ladder side rails did not extend at least 36 inches nor were the side rails tied off onto a sturdy structure. Employees were exposed to fall hazards from a ladder on or about June 25, 2015.
