29 CFR 1910.23(a)(8)(ii): Floor holes into which persons can accidentally walk were not guarded by floor hole covers of standard strength and construction. While the cover was not in place, the floor hole was not constantly attended by someone or protected by a removable standard railing: Rig 892: On or about June 25, 2015 a floor hole on top of the trip tanks was not guarded by floor hole covers. This condition exposed employees to a trip hazard.

Rig 892 Drilling Floor: On or about June 25, 2015 all ladders were not maintained in a safe condition, the derrick ladder was damaged and not secured to the rig floor. This condition exposed employees to a trip hazard.
