Worksite located at 5775 NW 64th Street, Kansas City, Missouri- Employee was observed working directly beneath an elevated employee putting on siding and was not protected from impact by the use of any means of head protection.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  (a). Worksite located at 5775 NW 64th Terrace, Kansas City, Missouri- Employees were exposed to shock or electrocution hazards in that some of the cords being used were frayed, missing ground pins and one cord was plugged in to a non-GFCI outlet.

Worksite located at 5775 NW 64th Terrace, Kansas City, Missouri- Employee was observed working in an aerial lift in which he was not wearing a body belt or  harness with a lanyard attached to the boom or basket of the lift.
