29 CFR 1910.1030(g)(2)(i): The employer shall train each employee with occupational exposure in accordance with the requirements of this section. Such training must be provided at no cost to the employee and during working hours. The employer shall institute a training program and ensure employee participation in the program.      (a)   Beauty Brands LLC., at 1270 E 1st Avenue, Broomfield, CO: On and preceding 06/23/15, the employer did not ensure that training was provided to employees with occupational exposure at the time of initial assignment to tasks where occupational exposure might take place. Nail technicians performed manicure work upon clients. Employees were occupationally exposed to blood during this activity. The employer had not provided training on bloodborne pathogens to employees. This condition exposed employees to a bloodborne pathogen hazard.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    (a)  Beauty Brands, LLC., 4600 Madison Avenue, Suite 400, Kansas City, MO 64112: The employer did not ensure that a written hazard communication program was developed and implemented for the establishment.  This condition exposed employees to chemical hazards associated with the use of chemicals such as but not limited to:    1. Perfect Color Sculpting Powders which contains methacrylate copolymer blends and titanium dioxide. Possible hazards associated with use of this product include eye irritation, respiratory irritation, and contact dermatitis; and      2. Nailprime Acid-Free Primer which contains ethanol. Possible hazards associated with use of this product include eye irritation, respiratory irritation, and contact dermatitis.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets. (a) Beauty Brands LLC., at 1270 E 1st Avenue, Broomfield, CO: On and preceding 06/23/15, the employer did not determine if employees are exposed to formaldehyde at levels above the OSHA Permissible Exposure Limit. Nail technicians performed manicure work upon clients. Employees were occupationally exposed to formaldehyde during this activity. This condition exposed employees to a chemical health hazard associated with formaldehyde exposure.
