29 CFR 1926.416(a)(2): In work areas where the exact location of underground electric powerlines is unknown, employees using jack-hammers, bars, or other hand tools which may contact a line shall be provided with insulated protective gloves: 15 Maple Tree Ct. Elmhurst, Illinois - Employees hand-dug and used shovels to expose the exact location of underground electric powerlines and were not provided with insulated protective gloves. Additionally, employees pulling copper piping from excavations with metallic couplers were exposed to indirect contact with buried energized lines and were not provided with insulated protective gloves. Employees were exposed to hazards likely to cause burns and/or electrocution. Abatement documentation in required for this item in accordance with the requirements of 29 CFR 1903.19(d)

29 CFR 1926.651(b)(3): When excavation operations approach the estimated location of underground installations, the exact location of the installations shall be determined by safe and acceptable means: 15 Maple Tree Ct. Elmhurst, Illinois - Employees hand-digging with shovels to expose the exact location of underground installations were not required to do so by safe and acceptable means. Additionally, employees pulling copper piping from excavations with metallic couplers and winches were exposed to indirect contact with buried energized lines. Employees were exposed to hazards likely to cause burns and/or electrocution. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 903.19(d).
