On or about June 25, 2015, the employer did not perform atmospheric testing prior to requiring employees to perform work on pressurized natural gas lines in an excavation that was greater than five feet in depth.

On or about June 25, 2015, the employer did not ensure excavated material from an excavation was at least 2 feet from the edge while employees worked in the excavation. The employees were exposed to cave-in hazards and struck-by hazards.
