While hand placing and holding a metal bracket in the point of operation of the press, the operator activated the foot pedal which resulted in the amputation of two fingers on his right hand.  The light curtains connected to the press brake were determined to be out of adjustment and did not prevent the press from cycling while the operator had his hands in the point of operation.
