29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Bar Stock Warehouse, on or about 7/31/15: A Berhinger horizontal bandsaw that is used to cut metal stock was not equipped with an adjustable guard to prevent contact with the unused portions of the blade during operation.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded: a) Brake Press Department, on or about 6/23/15: BP-2 Accupress was not effectively guarded to prevent a point of operation injury. While hand placing and holding a metal bracket in the point of operation of the press, the operator activated the foot pedal which resulted in the amputation of two fingers on his right hand. The light curtains connected to the press brake were determined to be out of adjustment and did not prevent the press from cycling while the operator had his hands in the point of operation.
