29 CFR 1910.145(f)(3): Use. Tags shall be used as a means to prevent accidental injury or illness to employees who are exposed to hazardous or potentially hazardous conditions, equipment or operations which are out of the ordinary, unexpected or not readily apparent. Tags shall be used until such time as the identified hazard is eliminated or the hazardous operation is completed. Tags need not be used where signs, guarding or other positive means of protection are being used. a. Oven Exit Line/Valve for Hot Water Hose - On or about April 17, 2015, a tag/sign was not placed to identify the hazard on a hot water hose's valve to prevent accidental splashes, thereby employees were exposed to thermal burns.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. a. East side of Building/Oven Exit Area - On or about June 26, 2015, the junction box for the conveyor's line motor was missing its cover plate, thereby exposing employees to electric hazards.
