29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: (a) Jobsite: On or about June 26, 2015, an employee was working on a steep roof (6 in 12)and was not protected from falling to the ground. The fall hazard to the employee was about 30 feet.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill: (a) Jobsite: On or about June 26, 2015, an employee who was working on a steep roof and was not protected from falling needs to be retrained so that he retains the skills and knowledge needed to minimize fall hazards while working on a roof. In addition, this employee was wearing a safety harness but it was not properly secured. The fall hazard to this employee was about 30 feet.
