29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a) Jobsite:  On or about June 26, 2015, employees who were working from two elevated platforms were not protected from falling about 21 feet to the ground.  These employees were handing up 4 foot by 8 foot pieces of particle board to employees on the roof.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    (a)  Jobsite:  On or about June 26, 2015, six employees were working from a steep roof (pitch is 5 in 12) and they were not protected from falling.  The fall hazard to the six employees was about 30 feet.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill:  (a) Jobsite:  On or a bout June 26, 2015, employees needed to be retrained to retain the knowledge and skills needed to minimize the fall hazards while working on a roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:       (a) Jobsite:  On or about June 26, 2015, employees were using half of a 32 foot aluminum extension ladder to gain access to the roof trusses and from there the employees would climb up the trusses and through a hole in the roof to access the roof.  This was being done in two places on the jobsite.  The fall hazard to the employees was up to approximately 16 feet onto a concrete floor.
