29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: On or about June 23, 2015, at the jobsite located at 3515 Broadway, Great Bend KS: An employee engaged in carpentry activities in the MRI room was exposed to fall hazards of approximately 24 inches to the ground below while entering and exiting the personnel point of access in that stairs were not provided where the break in elevation exceeded 19 inches.
