29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolding was not fully planked or decked:    a) Jobsite: The employer did not ensure that all working levels of the supported tubular welded frame scaffold were fully planked or decked between the front uprights and the guardrail supports.  An employee was demolishing the face of the building from the three levels of the scaffold, up to approximately 19 feet above ground level.    Observed on or about 6/23/15.

29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used.       a) Jobsite: The employer did not provide a safe means of access from one level of the supported tubular welded scaffold to another by the use of portable ladders, hook-on ladders, or other acceptable means.  An employee climbed the scaffold supports to access the three working levels.    Observed on or about 6/23/15.

29 CFR 1926.451(f)(6): The clearance between scaffolds and power lines were less than 10 feet.        a) Jobsite: The supported tubular welded frame scaffold was erected within 1 foot of an unguarded electrical wire exposing an employee to electrical shock hazards. The employee was conducting demolition activities from the end frame of the 3rd working platform adjacent to the electrical wire.      Observed on or about 6/23/15.
