29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: (a) On or about May 6, 2015, and at times prior, the employer did not ensure that working surfaces and walkways were free of debris that creates slip trip and fall hazards inside the mechanical room where pipe installation was occurring.
