Protection by placing and keeping such materials or equipment at least 2 feet from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary was not provided. Hanger 5 Renovation, MCAS Miramar, San Diego, CA: On or about 06/04/2015, the employer did not provide protection from equipment steel crowbar and a laser level rolling or falling into the excavation as set forth in the regulation above exposing the employee to severe head injuries.
