29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about June 3, 2015, employees were exposed to fall hazards of approximately ten (10) feet without wearing personal fall arrest systems while performing loading procedures on top of tank trailers. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
