29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about June 24, 2015, the employer failed to ensure that employees working from the employer made man box were protected from falling to the ground by use of personal fall protection devices or guardrail systems. The employer provided a large work box which was elevated by the forks of an industrial powered vehicle to reach the roof level. Employees worked both in the equipment and exited it to perform carpentry work. This hazard with respect to a worksite located at 9471 Market Street, North Lima, OH.
