29 CFR 1910.147(a)(2)(ii)(A): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee (s) was required to (A) Remove or bypass a guard or other safety device; (B) were required to place any part of his or her body into area on a machine or piece of equipment where work was actually performed upon the material being processed (point of operation) or where an associated danger zone existed during a machine operating cycle:   On or about June 24, 2015, an employee engaged in maintenance activities at the 2nd Brake C in the flour mill was exposed to caught-between hazards in that procedures were not developed and implemented which required the deenergization of the 2nd Brake B prior to removing the combination guard which covers the belt and pulleys of the two Brakes.
