29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Employees installing a standing seam metal roof on a new doctor's office were exposed to a fall hazard of approximately 32 feet to the ground below while accessing and egressing from a ladder that was not extended at least 3 feet.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  Employees were not wearing hard hats and exposed to struck by hazards to the head while installing a standing seam metal roof on a newly constructed doctor's office.      Mr. Marcos Alex Aguilar was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 881823, citation number 1, item number 1 and was affirmed as a final order on February 14, 2013, with a final abatement date of February 28, 2013, with respect to a workplace located at 5321 W. University Drive, McKinney, Texas 75071.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  Employees installing a standing seam metal roof on a new doctor's office were exposed to a fall hazard of approximately 32 feet to the ground below without fall protection.  Mr. Marcos Aguilar was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 881823, citation number 01, item number 002 and was affirmed as a final order on February 28, 2013, with respect to a workplace located at 5321 W. University Drive, McKinney, Texas 75071.  Mr. Marcos Alex Aguilar was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(2)(i), which was contained in OSHA inspection number 924600, citation number 01, item number 001 and was affirmed as a final order on November 5, 2013, with respect to a workplace located at 5151 Samuell Blvd, Dallas, TX 75229.
