The following hazards were noted during this inspection, many of which existed over the five days:    a/  Lack of adequate footwear.    b/  Lack of protective head gear.     c/  Lack of safety glasses.    d/  Lack of a self-closing lid on two portable containers partially filled with gasoline.     e/  Lack of conventional fall protection at unprotected, open sided edges inside of the structure.    f/  Lack of conventional fall protection during leading edge work.     g/  Using a portable, flexible cord not designed for hard or extra hard usage.     h/  Misuse of an extension ladder.     i/  Using the top step of an extension ladder for access to and egress from the second floor.

These employee(s) were exposed to the following fall hazards while framing a two-story residential home:    a/  Unguarded wall opening located along the south side of the residence and measuring approximately 10'5"W X 82"H X 9' above the ground below.    b/  Unguarded floor opening located along the east side of the residence on the second floor and measuring approximately 4'W X 8'L X 9' above the first floor.    c/  Sheathing the second floor where four, open-sided edges existed.  The fall distance to the ground below was approximately 16 feet.

These employee(s) were exposed to the following fall hazards while framing a two-story residential home:  a/  The exposed employees climbed down a green, portable step ladder that was positioned in the closed position.  b/  The exposed employees climbed down from the second story of the residence under construction and then placed their two feet on the top rung of a second step ladder before climbing down to the first floor.
