The employees were framing a residential home and were exposed to falls of 11 feet 2 inches at the front of the house to the ground and the employer did not ensure that the employees were provided with a PFAS (personal fall arrest system).

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: (a) Stonewood Creek Subdivision: 202 Stonewood Creek, Dallas, GA.: The employees were framing a residential home using a 7-1/4 inch Porter Cable Circular Saw (15 amps) and strain relief was not provided at the terminal end of the flexible power cord, exposing employees to possible electrical shock hazards.
