29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, were not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: At the east side exterior of the project, debris was not kept cleared from stairs as there were pieces of lumber, tools, and flexible cords on the stairs with six risers. Employees that used the stairs to access the interior of the building were exposed to the hazard of falling onto and down the stairs.

29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: Near the east side exterior of the project, eye protection was not worn while using a 110 VAC Milwaukee circular saw to cut lumber. The prescription eye wear worn was not marked with Z87. Non-prescription safety glasses were onsite. However, they did not fit over the prescription eye wear, and the exposed employee said he could not see well enough to do the work without his prescription eye wear. Employees were exposed to the hazard of being struck in the eyes by wood splinters.

29 CFR 1926.404(b)(1)(i): The employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. These requirements are in addition to any other requirements for equipment grounding conductors: Near the east side exterior of the project, there was no assured equipment grounding conductor program or ground-fault circuit-interrupters to protect employees that used a 110 VAC Milwaukee circular saw and a Bosch jig saw to cut lumber. Power to the saws was provided through an extension cord plugged into a 110 VAC receptacle in an exterior wall. The receptacle was not a ground-fault circuit-interrupter receptacle. Employees were exposed to the hazard of contacting electricity.

29 CFR 1926.1052(c)(1)(i): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail: At the east side exterior of the project, there was no handrail for the stairway with six risers. Each riser was 6.5 inches high. Employees were exposed to the hazard of falling onto the stairs, tools, and debris on the stairs.
