R & J Recycling, LLC., 1800 Garfield Ave. Parkersburg, WV: On or about 6/30/15, repairs made to oxygen and fuel gas burning hoses were not pressure tested to at least 300 p.s.i. ABATEMENT CERTIFICATION IS REQUIRED.
