29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) On or about June 30, 2015 and at times prior; the employer did not ensure a fall protection method was implemented and/or used while an employee was working at the roof area which exposed the employee to falls of 12.3 feet from the eave to the ground below.
