29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence:    (1) Know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy,   (2) Turn off and shut down the machines,  (3) Physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,  (4) Affix lockout or tagout devices to each energy isolating device,  (5) Prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergization of the machine or equipment have been accomplished.    Employees who performed servicing and maintenance did not lock out while changing tooling. The employees were exposed to struck-by, caught between and amputation hazards from unexpected start-up and energizing of the following CNC machine(s) when an orderly shutdown was not carried out:       1. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    The employer did not use adequate machine guarding to protect employees. Employees were exposed to cuts, lacerations, fractures and amputations when employees open the door to the following machines while they are running:        1. Okuma LB351-M Serial 13687  2. Okuma L1420 Serial 0565    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).    Johnson Pattern and Machine Works, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 802189, citation number 1, item number 3 and was affirmed as a final order on July 2, 2013, with respect to a workplace located at 350 W. Marquette Street Ottawa, Illinois 61350.
