29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: On or about June 30, 2015, employees performing work from forklift elevated work platforms at heights of 24 feet and less were exposed to the hazard of falling, and the employer did not obtain the manufacturer's prior written approval for the addition of the work platform. The Branding Iron Holding, Incorporated was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(a)(4) , which was contained in OSHA inspection number 995668, citation number 1, item number 2 and was affirmed as a final order on 12-3-14, with respect to a workplace located at 1682 Sauget Industrial Blvd, East Saint Louis, IL 62208.
