29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  a)      Admin Area West wall;  An outlet did not have a cover.  b)      1st Park Storage Building;  An outlet did not have a cover and a heater/AC was plugged into the outlet.  c)      2nd Park Storage Building;  Two outlets on the ceiling did not have covers.  d)      Park Storage Building #2909;  A junction box was missing a cover.  e)      Park Storage Building #2909;  An outlet on the ceiling was missing a cover.

29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail (approximately half way between the top rail and floor (21 inches)) and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor:  a)      Automotive and Welding Shop; The second floor storage area had a top rail which measured 41 inches high and a mid-rail which measured 12 inches high.  The midrail was not located half way in between the floor and top rail.  The Army Corps of Engineers Mud Mountain Dam was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 314899196, citation number 1, item number 2b and was affirmed as a final order on July 12, 2011, with respect to a workplace located at Enumclaw, WA.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a)       Woodworking Shop;  An office type of power strip (relocatable power tap) was used to charge Dewalt batteries and Pasiode batteries in a dusty shop environment. This violates the UL listing and labeling of the power strip and exposes employees to possible electrical fire hazards.

29 CFR 1910.303(g)(1)(i): Work space was less than 30 inches wide in front of electric equipment operating at 600 volts, nominal, or less:  a)  1st Park Storage Building;  The breaker panel was inaccessible due to storage of large boxes and metal caging.  b)  Metal Quonset, West wall;  The breaker panel was inaccessible due to storage of rock core samples leaving an 11 inch aisle way to access the panel.  c)  Metal Quonset, East wall;  Metal sheeting was stored underneath the breaker panel.
