29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:     On May 12, 2015, employees were not provided with a personal fall arrest system when operating an aerial lift, exposing employees to a fall hazard of approximately 22 feet.       Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.     On May 12, 2015, employees were not protected from a fall while walking/ working from the roof of a commercial building, exposing employees to a fall of approximately 22 feet.       Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:     On or about May 12, 2015, employees working at heights were not trained to recognize or minimize the hazards associated with a fall, exposing employees to a fall hazard of approximately 22 feet.       Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
