29 CFR 1926.601(b)(4)(i): The employer operated motor vehicle equipment with an obstructed view to the rear that was not equipped with a reverse signal alarm audible above the surrounding noise level: On or about June 29, 2015, at the jobsite located at Anson Rd. south of US HWY 160 in Wellington KS: An employee engaged in highway construction activities was exposed to struck-by hazards while picking up material near the railroad tracks in that a Chevrolet 2500HD utilized at the jobsite included an obstructed view to the rear and was not equipped with a backup alarm.
