29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: (a) An annual/periodic inspection of the energy control procedures has not been conducted for, but not limited to sewing machines, automated cutting machine, STAFAST T-Nut machine and other production equipment to ensure that procedures and requirements of the standard are being followed and that employees who implement energy control procedures understand their application.

29 CFR 1910.1200(h)(3)(iv): The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information: (a) Employees in production operations and maintenance are exposed to but not limited to paints, solvents, lubricants and MDI chemical. Training was not provided regarding the new label elements (product identifier, signal word, pictogram, hazard statement(s), and precautionary requirements) and new safety data sheet (SDS) format.
